Some patients have an issue with IgA antibodies which will cause severe side effects with immune globulin. Gammagard S/D has the lowest level of IgA and that is the product you should have been given. Other than that, all side effects with immune globulin are rate related. That means that when you give it too fast, you will have side effects. Any first dosing of IVIG (intravenous immune globulin) should be given at a very low rate. Not what the manufacturer says is OK which is too high for initial dosing. Most physician offices and/or infusion suites need to get you in and out. It is a big money maker for them. Home infusion with a qualified infusion nurse is the safer route. The same thing is going to be done if you have a big side effect-call 911. There are other things that can be done to enhance a better outcome for those with side effects. Intravenous fluid pre-hydration, low dose steroid 30 minutes before IVIG (if not diabetic or allergic), benadryl, tylenol and these started a day in advance. I have know patients to have horrible side effects in the beginning to being able to take the infusion with no problem in several months.
